KEY HOLDER CONTACTS

The Fire District requests key holder information for all occupancies with fire protection systems so that a representative can be contacted for activations and service issues.  The information is entered into a database at Ducomm, our 9-1-1 dispatch center.   To provide updated key holder information, please complete the form below (Note: At this time, the document is not form fillable - legible handwritten info is acceptable.) and email it to info@wegofpd.org or mail to 200 Fremont, West Chicago 60185.
