The West Chicago Fire Protection District fully supports and complies with the updated Freedom of Information Act (5ILCS 140/4). The District aims to operate in an accountable, transparent, and open fashion. The public has the right to information produced by their government, with certain privacy limitations. It is the policy of the Fire Chief’s office to aid in obtaining that information.

EMERGENCY MEDICAL RECORDS/PATIENT CARE REPORTS
**Please note that requests for medical records and copies of invoices are not handled by the West Chicago Fire Protection District, as we are not the custodian of these records.**

AMBULANCE BILLING
For ambulance billing inquiries, please contact Paramedic Billing Services at 630-530-2988, our third-party billing agency.

GENERAL PUBLIC RECORDS/FOIA REQUESTS (*Not a request for emergency medical or billing records)
If you are unable to locate the records you are requesting directly on our website (i.e., District meeting minutes, etc.), please complete the electronic request below by clicking the "FOIA ELECTRONIC REQUEST" LINK and then submit. Your response will be processed in accordance with the Illinois Freedom of Information Act.
FOIA ELECTRONIC REQUEST
Alternate request method: If you are unable to complete our electronic request, please submit an email to info@wegofpd.org and the following within in the BODY of the email:
1) Name of Record Requestor
2) Email of Record Requestor
3) Direct Contact Telephone Number for Record Requestor
4) Description (with detail if possible) of records being requested, including address (addresses) related to the records, a start and end timeframe, or specific date/time included.
5) Please also include the term "FOIA" in the SUBJECT LINE, with a brief title, so the request can be processed without delay.
*PLEASE NOTE: In most cases, the records associated with the FOIA request will be returned via the email listed as "Email fo Record Requestor".
